A Paradigm Shift in Dupuytren's Disease

New insights into Dupuytren's disease have triggered a paradigm shift in treatment in recent years:

Key Insights into Dupuytren's Disease

  1. Genetic Disposition and Trauma: In addition to individual genetic predisposition, any blunt or sharp trauma to the hand can lead to the progression of Dupuytren's disease.
  2. No Curative Treatment: A complete cure for Dupuytren's disease is not possible with any of the currently available therapies.
  3. Open Surgery: Open surgery with skin incisions is highly traumatic. It leads to extensive scarring and complicates future interventions.
  4. Needle Fasciotomy (PNF): Needle fasciotomy (PNF and, if necessary, PNF+) can be used to treat every stage of Dupuytren's disease.
  5. Repeatability of PNF: PNF and, if necessary, PNF+ can be applied repeatedly without increased side effects.
  6. Treatment of Finger Contractures: Early finger contractures, from 10 degrees upward, can be effectively stretched using the Push Hand Orthosis DPT.
  7. Advanced Stage of Dupuytren's Disease: In advanced stages of the disease, the Push Hand Orthosis DPT after PNF or PNF+ provides more sustainable results and may improve any remaining residual curvatures in the middle joints.

The Demand for a Minimally Invasive Therapy

The paradigm shift in Dupuytren's disease highlights the need for minimally invasive and easily repeatable therapy.

The Importance of the Push Hand Orthosis DPT

The Push Hand Orthosis DPT for finger straightening is an essential component in the therapy of Dupuytren's disease. It not only improves the outcomes of minimally invasive treatments but also enhances the sustainability of any surgical therapy for contracted fingers.

Information for Dupuytren's Patients

Dear Dupuytren patients,

As an experienced surgeon I have used needle fasciotomy as a therapy for Dupuytren’s disease for 30 years since 1994.
In 2012, I organised a work group in Nürnberg under Dr. Wolfgang Wach and the patronage of the Deutsche Dupuytren Gesellschaft to enable 6 colleagues to share their experiences on needle fasciotomy.

Since giving up my concession to practise under the german health insurance system in 2020, I have been practising PNF, exclusively, as a private doctor up until 2024.

A new approach: Needle Fasciotomy and Dynamic Splinting

Traditional open surgery with its skin incisions should no longer be used as a therapy for Dupuytren’s disease. Instead, there is an effective synergistic combination of needle fasciotomy and dynamic splinting for treating every stage of Dupuytren’s disease.

The Push Hand Orthosis DPT

In October 2024 a new dynamic finger extension splint, the Push Hand Orthosis DPT, will be marketed, initially in Bielefeld. This innovative therapy was presented and was awarded a prize at the International Dupuytren Symposium in Oxford on 01.12.2021. The presentation can be viewed on You Tube.

Certification and Application of the DPT Orthosis

Certification of medical supply stores by the manufacturer is required, as the DPT orthosis is not a therapy-supporting bandage, but an independent therapy for the treatment of Dupuytren's disease. Although patients can individually decide on the duration and frequency of application, professional guidance in the medical supply store is essential.

Private and Insurance Prescriptions for the DPT Orthosis

My patients can receive a private prescription for the DPT orthosis from me. Statutory health insurance patients have the option to have this prescription converted into a statutory health insurance prescription by their general practitioner, but they should expect a copayment, the amount of which will vary depending on their health insurance provider.
